Between discharge and the first aftercare appointment lie the days when contact breaks off.

On the day of discharge, it is clear which forms of aftercare are possible, but not whether the requirements are met or who books the first appointment. Social services give advice verbally, note the outcome in the discharge letter and hand the file on. The facility usually never finds out whether the patient turns up the following week.

4forms of aftercareOutpatient follow-up treatment, rehabilitation aftercare, individual therapy and self-help are all options.
3requirements per formThe payer, place of residence and waiting time decide what is possible at all.
40 minutesadvice sessionThat is how long it takes to go through the options verbally on the day of discharge.
14days without contactBetween discharge and the first aftercare appointment, nobody gets in touch.

The 14 days without contact after discharge give way to a sheet setting out the form, the requirements and the first step, and 45% more aftercare placements follow.

The 45% is this agent's headline figure. The number of forms, the length of the session and the days without contact reflect a typical discharge; they are not measured values.

Three steps through the process.

The agent matches the possible forms of aftercare, checks their requirements and prepares the transition; therapy management is responsible for the recommendation.

Match forms of aftercare to the case

The agent reads the diagnosis group, payer and place of residence from the record and lists the forms of aftercare that could apply. For each form, it names the requirement still missing.

Case NS-2026-0338, 3 of 4 forms possible, 1 requirement open

Check requirements one by one

For each form, the agent checks whether the payer is responsible, how long the wait is and how easily it can be reached from the patient's home. It records where each piece of information comes from.

Review note with 12 points, 22 minutes' travel to the day clinic

Initiate the transition in writing

The agent prepares the letter to the aftercare provider and an information sheet for the patient. Social services add anything relevant from the discharge conversation and approve it.

What the agent does not do.

It does not make a treatment recommendation.

It sets out the possible forms with their requirements. Therapy management decides what to recommend.

It does not book appointments.

It prepares the letter and the proposed appointment. The aftercare provider or social services make the call.

It does not follow the patient after discharge.

It only sees whether the first appointment was kept if someone notes it in the record.

The questions that come up before the first meeting.

How quickly is the Aftercare Adviser Agent ready to use?

A pilot typically runs on your own data within 4 to 6 weeks. The agent connects to your existing systems rather than running as a separate, parallel environment.

Where is the data processed?

EU cloud, private cloud or on-premises. Processing is GDPR-compliant and models are never trained on your data. Details in the Trust Center.

Where does the figure “45% more aftercare placements” come from?

It is based on experience from comparable projects and process benchmarks, not a commitment. We work out the realistic effect for your organisation during the assessment before the pilot.
