Care Planning Agent

The shift ends with half an hour of copying up notes from scraps of paper. Whatever is missing there is missing from the handover.

Helps nursing staff keep documentation in order and plan shifts.

A shift's documentation is written after the shift, from memory.

Interventions, vital signs and observations are noted at the bedside and transferred to the nursing documentation later. Alongside this, tasks are checked against the roster, because open tasks pass to the next shift. If an intervention goes unrecorded, it only resurfaces on the ward round.

4open tasksWhatever was not done goes into the handover verbally and is never written down.

At the end of the shift, the 34 entries are already assigned and the half hour of catch-up disappears: 30% less documentation effort.

The 30% reduction in documentation effort is this agent's headline figure. Patient numbers, entries and catch-up time describe a typical early shift on a ward; none of it was measured here.

Three steps through the process.

The agent structures the nursing documentation and checks open tasks against the roster; it makes no judgement on the care intervention itself.

Structure a shift's entries

The agent assigns notes, vital signs and interventions to the fields of the nursing documentation. Each entry keeps its time and the nurse who made it.

Check tasks against the roster

The agent checks open tasks against the staffing of the next shift. It names whatever remains unassigned.

4 open tasks, 3 assigned to the late shift, 1 without a nurse

Pass the handover on for signature

The agent presents the structured documentation and the handover list for review. The shift coordinator adds to them and signs.

What the agent does not do.

It cannot document what nobody tells it.

It organises what has been recorded. If an intervention is missing, the line stays empty and the nurse fills it in.

It does not order any care intervention.

It carries open tasks forward and shows who can take them on. Nursing management orders the intervention.

It does not change the roster.

It works with the staffing that has been entered. The nursing shift coordinator makes any reassignments.

The questions that come up before the first meeting.

How quickly is the Care Planning Agent ready to use?

A pilot typically runs on your own data within 4 to 6 weeks. The agent connects to your existing systems rather than running as a separate, parallel environment.

Where is the data processed?

EU cloud, private cloud or on-premises. Processing is GDPR-compliant and models are never trained on your data. Details in the Trust Center.

Where does the figure “30% less documentation effort” come from?

It is based on experience from comparable projects and process benchmarks, not a commitment. We work out the realistic effect for your organisation during the assessment before the pilot.
